Change of Address

Please notify us immediately of any change of name or address or telephone number, so that we can keep our records accurate.

New Patients

When you register with this practice you will be asked to make an appointment for a health check with our Practice Nurse.   This includes a health questionnaire and measurement of weight and blood pressure.  The health check gives us valuable information about your medical background.

Chaperone Policy

It is our policy to offer a chaperone to any of our patients when having an examination.  If a suitable member of staff is not available at the time of your appointment, you may be requested to re-arrange this appointment.
