When you register with the practice you will be asked to make an appointment for a health check with our Practice Nurse. This will include a health questionnaire and measurement of weight and blood pressure. The health check gives us valuable information about your medical background.

If we do not have this information in full your registration will be declined. Also when completing this form there is an ethnicity form attached, (which must be completed and ticked where applicable). Please can you put a daytime contact number as well as a home number.
In order to register with a GP Surgery, you are now required to provide proof of your address AND proof of identity.

IF YOU ARE REGISTERING A CHILD
You will need to provide proof of identity in the form of a Birth Certificate
IF YOU ARE FROM THE EU AND NOT A UK RESIDENT
You must provide proof that you intend to stay longer than six months, i.e. work contract, tenancy agreement (which must be in your name)
IF YOU ARE FROM OUTSIDE THE EU
We must have sight of your passport and entry visa, which must be valid for the minimum of a further six months. We will also need proof of your address as above.
IF YOU ARE AN OVERSEAS STUDENT
We will need a letter from your university, college or school stating that you are in full time education and confirming the course you are attending. Passport and entry visa are needed.
If you are returning from university and were previously a patient at this practice, you will not be required to produce any of the above.
IF YOU ARE AN AU-PAIR
We need to see your passport. A letter from your employer is also required, which must confirm the date you started in their employment and approximately how long they expect you to continue working for them.
